since i need the database proxy service running very infrequently, i often forget the steps required to enable it.
so here they are:

• in connection administration->system settings->advanced->connection administration
  database proxy: service shutdown timer needs to have a value greater than 0
• unity connection serviceability – log in with application administrator account
  activate the connection database proxy service and make sure the status changes to started
